ABSTRACT

ABSTRACT: Human identification is the process that identifies an individual as a unique being, differentiating him/ her from all others. The participation of Odontology in the processes of identification is fundamental in situations of human remains. This paper aims to report a case of positive identification, through dentistry, in human remains. The forensic odontologist performed a post-morten (PM) examination, which consisted of the collection of post-mortem dental findings in human bones, collection of ante-mortem (AM) dental findings in the patient's orthodontic records, and the comparison between AM and PM information. Dental elements are widely used in the processes of human identification, since they are resistant to time and to physical, chemical and mechanical impacts. Positive identification is guided by the presence of individualizing characteristics, easily detected in dental records. It is concluded that human identification through dentistry is a safe and reliable method, since dental elements have individualizing characteristics that make the process possible.

ABSTRACT

Florid osseous dysplasia (FOD) is a fibrous lesion relatively uncommon of maxillary bones, mainly affects middle-age black women and generally is diagnosed on routine radiographs. This paper aims to report a case of black woman, aged 50 years, referred to Dentistry Faculty of Federal University of Ceará for imaging evaluation. Clinically, there were no facial or intraoral structures alterations. Panoramic radiography showed radiopaque images circumscribed by radiolucent halos, located bilaterally in the edentulous mandibular body region suggestive of FOD. Cone beam computed tomography (CBCT) showed hyperdense areas in edentulous mandibular regions, with no expansion of buccal and lingual cortical bone, and covered by fine thickness of bone tissue, characteristic of FOD. After the clinical-imaging diagnosis of FOD, the patient was referred for multidisciplinary dental treatment. Through the CBCT, it was possible to establish a treatment plan that consisted of minimally interventional procedures considering the clinical and imaging limits of the observed lesions. In summary, the present case emphasizes a meticulous clinical examination along with an evaluation by means of CBCT allowing an adequate treatment plan for asymptomatic FOD... (AU)

ABSTRACT

Individuals with Obstructive Sleep Apnea (OSA) may present changes in the craniofacial skeleton and pharyngeal airway space that have an influence in sleep pattern and OSA severity. Radiographic cephalometry is an imaging test routinely used by orthodontists that allows assessment of the craniocervical facial anatomy including the pharyngeal airway spa-ce. Objective: To evaluate if the cephalometric anatomical components have any correlation with polysomnographic parameters of individuals with OSA. Materials and Methods: The study sample consisted of 50 individuals with polysomnographic diagnosis of moderate and severe OSA. All volunteers underwent a full-night polysomnographic evaluation and a lateral cephalometric radiographic examination. Results: Significant correlations were found between the polysomnographic variable Apnea and Hypopnea Index (AHI) and the cephalometric variablecorresponding to the width of the soft palate (p <0.01) and the distance from the hyoid to the third cervical vertebra (p <0.01). The polysomnographic variable Oxyhemoglobin Minimal Saturation (SpO2) showed a significant correlation with the two cephalometric variables mentioned above, as well as with the variable corresponding to the length of the soft palate (p<0,05). Conclusion: Some cephalometric variables correlate with polysomnographic parameters, but we cannot say that cephalometry can be used as a predictor of OSA severity. (AU)

ABSTRACT

Introduction: Sodium hypochlorite is a solution used in endodontic treatment, and if an accidental apical extrusion occurs, serious complications may affect soft tissue. Objective: The aim of this article is to present a case of apical extrusion of sodium hypochlorite (NaOCl) during root canal system instrumentation. Case report: A 28-year-old woman sought a local dental service for root canal treatment of tooth #24. Conventional endodontic treatment was adopted, which consisted of the use of 2.5% NaOCl solution as irrigation solution applied with a 10-ml syringe and 22G hypodermic needle. At that time, the patient reported extreme pain and a burning sensation in the left maxillary region, followed by the formation of intense edema. A clinical diagnosis of hypochlorite-induced cellulitis was made. The patient was treated immediately with amoxicillin (500 mg, orally) at intervals of 8 h for 7 days and dexamethasone (4 mg, intramuscularly) at intervals of 24 h for 3 days. In the subsequent endodontic treatment, 2% chlorhexidine gel applied with a 5-ml syringe and 24G needle was used as irrigation for a better control of treatment and to prevent new accidents. The root canals were instrumented using adequate crown-down technique, which provides a conical shape, and filled by lateral condensation technique. The patient was symptom free at the 8-month clinical follow-up. Conclusion: Dentists should always carefully follow all stages of dental treatment planning without neglecting any of them, paying attention to the solutions used and their storage as well as performing a specific technique with maximum accuracy. If an accident occurs, the appropriate handling of the situation will enable the satisfactory completion of treatment.

ABSTRACT

INTRODUCTION: The rarity of oral neurofibromas (ONs) generates problems regarding their epidemiological and immunohistochemical characterization. OBJECTIVES: The aim of this study was to evaluate the expression of different markers in ONs and review epidemiologic data reported in the literature. MATERIAL AND METHODS: Clinicopathologic, immunohistochemical (markers S-100, epithelial membrane antigen [EMA], CD34) and histochemical (modified-Ziehl-Neelsen-method) studies were performed in eight cases of ON diagnosed in the Department of Pathology and Legal Medicine (DPML), Universidade Federal do Ceará (UFC), Ceará, Brazil, between 1994 and 2010. RESULTS: Oral neurofibromas represented 0.2% of the oral lesions diagnosed by our service in 16 years, and the buccal mucosa was the most frequent oral site (71.4%). Seven (87.5%) and 8 (100.0%) cases were positive for S-100 and CD34, respectively, and none for EMA. Mast cells were identified in seven cases (87.5%). The literature search indicated that solitary ONs are more common and occur preferentially in females, affecting patients between 30 and 40 years old. The alveolar ridge is the most commonly involved site. CONCLUSION: S-100- and CD34 markers proved to be of great value as a diagnostic tool, unlike EMA staining. Identification of mast cells in most cases suggests their involvement in this tumor pathogenesis. The clinicopathologic data retrieved from the literature enabled the establishment of a more consistent epidemiological profile.

ABSTRACT

Salivary duct cyst (SDC) in an uncommon lesion of the minor salivary glands, with etiology related to obstruction of the salivary duct. It presents with a color similar to that of the mucosa, is mobile, measures between 3 and 10 mm and affects preferentially the bottom lip, fl oor of the mouth and the jugal mucosa. SDC have a predilection for the male and individual in elderly groups. The pathogenesis of SDC is associated with the formation of a mucous plug that causes partial or total obstruction of the salivary gland system duct, resulting in the dilation of the duct and increase in intraluminal pressure. They present clinical-pathological characteristics similar to those of salivary gland tumors, making diagnosis diffi cult and subject to errors in treatment. It is important for the dentist to include SDC in the differential diagnosis of lesions that affect the upper lip, although it is relatively rarely found in this anatomic site.

ABSTRACT

PURPOSE: To evaluate the interference of radiographic factors in the appearance of sensory deficit related to inferior alveolar nerve (IAN) after third molars (3Ms) removal. METHODS: A prospective, double-blind, observational, unicentric study was performed with 126 patients submitted to a surgical procedure of lower 3Ms removal in the period from March to October/2011. Collected data included gender, age, eruption stage of 3Ms, position/angle of 3Ms (Pell-Gregory and Winter classifications, respectively), presence/absence of radiographic signs of 3Ms proximity with the inferior alveolar canal and surgical technique. Occurrence evaluation of the IAN injury was performed on the seventh postoperative day through pin-prick, two-point discrimination and brush directional stroke tests. RESULTS: Predominant radiographic signs were: narrowing of the inferior alveolar canal (68.25%), darkening of root (46.82%) and diversion of the canal (31%). None of the patients presented sensory loss. Sixty-one (48.41%) of the cases had at least one or two radiographic signs of proximity with NAI. Forty-seven (37.3%) had 3 or more signs, and 18 (14.29%) did not have any radiographic signs of proximity to mandibular canal. CONCLUSION: There was not a positive correlation between presence of radiographic signs of 3Ms with IAN proximity and postoperative neurosensory disorders occurrence.

ABSTRACT

Cementoblastoma constitutes an odontogenic neoplasm, and is considered rare as it accounts for about 1% to 6.2% of all odontogenic tumors. This lesion has been described most commonly in young patients, with about 73% of cases occurring in those under 30 years of age. On the basis of its clinical and radiological features, it is characterized as a usually painless swelling, closely associated with the root of multiradicular teeth. Treatment is generally limited to the surgical removal of the lesion together with the tooth involved. The aim of this paper is to report a case of cementoblastoma treated by a combination of endodontic and surgical therapy.

ABSTRACT

PURPOSE: To study the main effects of local use of liquid nitrogen on bone marrow tissue in rats. METHODS: The femoral diaphyses of 42 Wistar rats were exposed to three local and sequential applications of liquid nitrogen for one or two minutes, intercalated with periods of five minutes of passive thawing. The animals were sacrificed after one, two, four and 12 weeks and the specimens obtained were analyzed histomorphologically. RESULTS: In the second experimental week of one-minute protocol, histological degree of inflammation obtained a mean score of one (mild), ranging from 0 (absent or scarce) and two (moderate) (Kruskal-Wallis test p=0.01). In the second experimental week of two-minute protocol, degree of inflammation to the medullar tissue obtained an average score of two (Kruskal-Wallis test p=0.01). CONCLUSION: The degree of inflammation of the bone marrow tissue was higher in protocol of three applications of two minutes compared to protocol of three applications of one minute.

ABSTRACT

OBJECTIVE: The aim of this study was to evaluate the effects of liquid nitrogen cryosurgery on the femoral diaphysis of rats. MATERIAL AND METHODS: The femoral diaphyses of 42 Wistar rats were exposed to three local and sequential applications of liquid nitrogen for 1 or 2 min, intercalated with periods of 5 min of passive thawing. The animals were sacrificed after 1, 2, 4 and 12 weeks and the specimens obtained were processed and analyzed histomorphometrically. RESULTS: The depth and extent of peak bone necrosis were 124.509 µm and 2087.094 µm for the 1-min protocol, respectively, and 436.424 µm and 12046.426 µm for the 2-min protocol. Peak necrosis was observed in the second experimental week with both cryotherapy protocols. CONCLUSIONS: The present results indicate that the 2-min protocol produced more marked bone necrosis than the 1-min protocol. Although our results cannot be entirely extrapolated to clinical practice, they contribute to the understanding of the behavior of bone tissue submitted to different cycles of liquid nitrogen freezing and may serve as a basis for new studies.
